A certificate indicating only the county will not be accepted.<\/li>\n<li>The certificate must include the\u00a0<strong>full names of both parents<\/strong>\u00a0and either their\u00a0<strong>dates of birth or their ages at the time of the child\u2019s birth<\/strong>.<\/li>\n<li>The certificate must be fully translated into Italian (see information on transactions below)<\/li>\n<\/ul>\n<p><strong>APOSTILLE:<\/strong><\/p>\n<ul>\n<li>The Apostille must be requested from the Secretary of State of the state in which the document was issued (e.g., if the birth certificate was issued by the Department of Health of Pennsylvania, the Apostille must be requested from the Pennsylvania Secretary of State).<\/li>\n<li>The Apostille must not be translated into Italian.<\/li>\n<li>The following link contains useful information and contact details for the Secretaries of State of the U.S. states covered by this Consular jurisdiction<\/li>\n<\/ul>\n<p><u>Useful informations on how to request an Apostille\u00a0<\/u><a href=\"https:\/\/consfiladelfia.esteri.it\/wp-content\/uploads\/2023\/05\/apostille_request_information.pdf\"><strong><u>(link)<\/u><\/strong><\/a><\/p>\n<p>&nbsp;<\/p>\n<p><strong>PARENTS WHO WERE UNMARRIED AT THE TIME OF A CHILD&#8217;S BIRTH<\/strong><\/p>\n<p>If the parents of the minor were NOT married to each other at the time of the child&#8217;s birth, they may provide an \u201cAffidavit of Parentage for child born out of Wedlock\u201d (or \u201cVoluntary Acknowledgement of Paternity\u201d, or equivalent) issued by the Department of Health of the State where the child was born.
