The passport will be voided and returned to the family (send a SASE\u00a0envelope\u00a0for the return of the voided passport);<\/p>\n<p>\u2022 Completed and signed application\u00a0<a href=\"https:\/\/consfiladelfia.esteri.it\/wp-content\/uploads\/2023\/05\/modulo_trascrizione_morte.pdf\" target=\"_blank\" rel=\"noopener\">FORM<\/a> to download!<\/p>\n<p><strong>2. DOCUMENTATION INFORMATION<\/strong><\/p>\n<p>\u2022 The death certificate can be obtained from the Department of Health of the State where the death occurred.<\/p>\n<p>\u2022 The certificate must indicate the city or town of death (the county is not sufficient)<\/p>\n<p><strong>APOSTILLE<\/strong>:<\/p>\n<p>\u2022 The Apostille can be requested at the Secretary of State of the State in which the document was issued (for example, if the birth certificate was issued by the Department of Health in\u00a0 Pennsylvania, the apostille must be requested to the Secretary of State of Pennsylvania).<\/p>\n<p>\u2022 The apostille does NOT have to be translated into Italian<\/p>\n<p>\u2022 The following link contains useful information regarding the U.S. Secretaries of State within\u00a0this consular jurisdiction\u00a0<a href=\"https:\/\/consfiladelfia.esteri.it\/wp-content\/uploads\/2023\/05\/apostille_request_information.pdf\" target=\"_blank\" rel=\"noopener\">Useful information on how to request the Apostille<\/a><\/p>\n<p><strong>TRANSLATIONS:<\/strong><\/p>\n<p>The translation must be complete and correspond to the order of the fields and data shown on the original, must be typed and must not show conversions of weights and\/or measures. Translations that do not meet these requirements will be rejected. The Consulate does not provide document translation services. Self-produced translations are acceptable if they are complete, accurate, and typewritten.
