- Photocopy of VALID ITALIAN ID of the Italian parent/grandparent (passport or identity card). If deceased, present the last valid document in their possession;
- FULL EXTRACT OF BIRTH CERTIFICATE (with indication of paternity and maternity and possible annotations) of the Italian parent/grandparent issued by the competent Italian municipality (issued within six months from the date of the birth registration request);
- HISTORICAL CITIZENSHIP CERTIFICATE issued by the competent Italian municipality (issued within six months from the date of the birth registration request);
- HISTORICAL RESIDENCY CERTIFICATE issued by the competent municipality of the last residence or AIRE registration of the Italian parent/grandparent (in the United States, the competent Authority for the issuance of this certificate is the U.S. Citizenship and Immigration Service, USCIS);
- Only if the Historical Residency Certificate indicates residences abroad: DECLARATION OF NON-NATURALIZATION*issued by the authorities of all the countries where the parent/grandparent resided.
- IMPORTANT:
*The DECLARATIONS OF NON-NATURALIZATION must be presented in the original and must be legalized and translated into Italian. Legalization through apostille is required if the country of issue has signed the Hague Convention of 05.10.1961. Certificates issued by countries that are not signatories to the Hague Convention of 05.10.1961 must be legalized by the competent Italian Embassy/Consulate.
The translation into Italian requires legalization/certification by the competent authorities:
- the Consulate General of Italy in Philadelphia, exclusively for the certification of translations of certificates issued by U.S. authorities (see below).
- For certificates issued by authorities in other countries, please, inquire with the Italian Embassy/Consulate of reference for the country where the certificate was issued.

This Consulate DOES NOT OFFER TRANSLATION SERVICES. We strongly advise you to contact a professional translator via internet or, alternatively, please contact one of the translators from this list (click HERE) . We reserve the right to refuse the certification if the translation is not faithful to the original.
IMPORTANT:
- all translations of certificates to be submitted to this Consulate must be certified following the instruction below. Letters of conformity written by the translators are not acceptable as a certification substitute.
- The certification of a translation can only be issued upon submission of the original apostilled document and for the following categories of documents only:
- Vital records and U.S. criminal background checks (apostilled) to be used with Italian public administration offices;
- Italian vital records and criminal background checks to be used with U.S. authorities.
U.S. certificates MUST always be legalized by means of Apostille. It is the applicant’s responsibility to request the legalization by the relevant authorities BEFORE requesting the certification of a translation. PLEASE NOTE: The Apostille must NEVER be translated, and therefore does not require translation’s certification
- For certificated issued by other countries and in languages other than Italian or English, applicants must contact the competent consular authorities in the country where the certificates were issued. For the certification of documents other than those listed above, it is recommended to contact a sworn translator registered with any Italian court.

To request certification of translations, the following documentation must be submitted:

- Photocopy of a valid ID of the applicant, including the pages showing the photo and signature
- Original certificates bearing an Apostille
- Payment by USPS Postal Money Order ONLY (do not send cash or checks), payable to “Italian Consulate General” (please write your full name on the Money Order). For services fees, click HERE
